Before You Come In

Doctor’s appointments and Insurance can be confusing. We want to make your experience as smooth and uncomplicated  as possible. We do not require a referral, but we suggest that you have one as many insurance companies require one to be seen. We are not your typical eye doctor. If you have been referred to us, it is because we provide unique services that aren’t available elsewhere. Due to the nature of the services we provide, some of them will not be billable to a health insurance policy or a few of the auto or workman’s comp companies. Most people should EXPECT some out of pocket expense. An estimate of your out of pocket costs will be discussed when you make your appointment. Be prepared to bring a driver’s license or photo ID and a copy of your insurance cards. Be sure to complete ALL of the requested paperwork and bring it with you to your appointment. If we will be billing Auto or Workman’s comp, please make sure that you have the correct claim numbers AND the correct name and contact info for your adjuster. 

We accept Priority Health, Blue Cross Blue Sheild/BCN, Medicare, Auto and Workman’s Comp.
